Return and Exchange Instructions

1. Fill out the Return and Exchange Form.

2. Print Page 1 of the Form twice before submitting it.

   • You can cut out the address on this page to use for your return shipment.
   • Include one copy of the printed page inside the package so we can identify your return.
     
     

3. Print Your Order Details – find your order here

4. Submit the Form.

   • After submitting, you will receive a copy by email. Our team will follow up with you as soon as possible.
     
     

5. Prepare Your Return Package:

   • Include the two printed documents (Page 1 and your order details) along with your LAKS wearable.
